OD is on in 4th, 5th, and 6th gears. It only carries about 60% of the input torque in 4th gear (it carries 100% in 5th and 6th), so if it neutrals out in 4th, that is unusual. That is, if your clutch is going south, usually it will first start slipping in 5th and 6th (where it carries more torque).

I am not hard on the truck and changed the fluid and filters at 100k. It doesnt make any attempt to move in drive, but also doesnt make any noises. IIRC if it was reflashed to factory and was in limp mode, tuning with some tuners might not take it out of limp mode.
